2001 Audi A4 - Who submits the proof of correction on a smog check when repairs were conducted by a smog mechanic?

After failing a smog check (at a test only site) I had repairs done and the car passed the smog check at an authorized smog check repair shop. The DMV record shows that the car passed the second smog test. However, the DMV records indicate that proof of correction is needed.

What is the proof of correction? Do I send it to the DMV or does the repair shop?

Proof of Correction is typically required when a vehicle receives a citation from a law enforcement official. The Proof of Correction is offered by the CHP (California Highway Patrol) or a local Sheriff's office, and must be submitted to the DMV by the vehicle's owner.
